Dear Dr Svoboda,

Thank you for accepting this referral for Mr John Beckham, a 71-year-old retired engineer, who presented today with
worsening lower urinary tract symptoms and a suspicious nodule felt on digital rectal examination. I would be grateful if
you could offer him an appointment for an urgent prostate ultrasound, a biopsy and a diagnosis.

Mr Beckham first became aware of lower urinary tract symptoms about three months ago. These now

worsening symptoms include frequency, hesitancy and nocturia. Mr Beckham has not noticed any blood in his

urine or semen and urinalysis did not indicate any urinary infection. Consent was gained for a digital rectal examination
and a hard, nodular prostate was felt. Prostate specific antigen levels were taken and I await these blood test results.

Mr Beckham has no previous medical or family history of note and no known allergies. He is not currently taking
any prescribed medication or herbal remedies, however, does take over-the-counter multivitamins daily. Mr
Beckham lives with his wife and enjoys gardening and cycling. He has a healthy diet and does not drink alcohol or smoke.
